Injury Liability Waiver

I, the parent/guardian understand that participation in basketball and related activities involves inherent risks, including but not limited to injury, illness, falls, collisions, or other accidents. I voluntarily assume all risks associated with my child’s participation in the B.E.S.T. Hoops Youth League. I hereby release, waive, and discharge B.E.S.T. Hoops Foundation Incorporated, its directors, coaches, staff, volunteers, and facility partners from any and all liability arising from injury, loss, or damage incurred during participation. I acknowledge that my child is physically able to participate and that I am responsible for any medical expenses that may arise.

Medical Authorization

In the event of an emergency, I authorize B.E.S.T. Hoops staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that reasonable efforts will be made to contact me as soon as possible.

Parent Code Of Conduct​

As a parent/guardian, I agree to support a positive environment for all players, coaches, referees, and families. I will encourage good sportsmanship, respect coaches and officials, avoid disruptive or aggressive behavior, and allow coaches to coach. I understand that failure to follow these expectations may result in removal from the facility and/or removal from the program without refund.

Player Code Of Conduct

The participant/player agrees to show respect to coaches, teammates, opponents, and officials at all times, demonstrate good sportsmanship, maintain a positive attitude, listen to coaches, and be coachable. Each player is expected to give their best effort during all activities and support their teammates in a positive and respectful manner. B.E.S.T. Hoops is committed to creating an environment where all players can learn, compete, and grow both on and off the court.

Photo & Video Release

I grant permission for B.E.S.T. Hoops Foundation Incorporated to use photos and videos of my child for promotional, educational, and marketing purposes, including use on social media, websites, flyers, and other program materials.​

Jersey Policy

I acknowledge that I have read and understand the B.E.S.T. Hoops Youth League Jersey Policy. I agree to pay a replacement fee of $75.00 per jersey if my child’s jersey is lost, damaged, or not returned to their coach by the designated deadline (End of the season)

I understand and agree that all jerseys issued by B.E.S.T. Hoops Youth League remain the property of B.E.S.T. Hoops Foundation Incorporated and are provided for temporary use during the season. I accept full responsibility for the care, condition, and timely return of all issued equipment. I further agree that B.E.S.T. Hoops Foundation Incorporated reserves the right to charge the card on file or otherwise collect the stated replacement fee if the jersey is not returned, returned late, or returned in damaged condition beyond normal wear and tear. By agreeing, I release B.E.S.T. Hoops Foundation Incorporated, its directors, coaches, staff, and volunteers from any claims or disputes related to the issuance, use, or return of league uniforms and acknowledge that all decisions regarding uniform condition and replacement are made at the sole discretion of the organization.
